Bestcef 400 is indicated for the treatment of:
Respiratory, Ear, and Urinary Tract Infections: 200–400 mg daily (single or divided doses).
Gonorrhea: 400 mg as a single dose.
Typhoid Fever: 20 mg/kg/day in two divided doses for 7–14 days depending on severity.
Children (<50 kg): 8 mg/kg/day every 12–24 hours.
6 months–1 year: 75 mg daily.
1–4 years: 100 mg daily.
5–10 years: 200 mg daily.
Typhoid Fever: 15–20 mg/kg/day divided every 12 hours for 7–14 days (Max 400 mg/day).
In renal impairment (CrCl <20 mL/min): The Maximum dose should not exceed 200 mg daily.
Less Common but Possible:
Probenecid increases cefixime concentrations.
May prolong prothrombin time when used with anticoagulants (e.g., warfarin).
Cefixime binds to penicillin-binding proteins (PBPs) and blocks the transpeptidation step in bacterial cell wall synthesis, leading to bacterial cell death.
Hypersensitivity to cefixime or other cephalosporins.
Pregnancy: Available studies suggest no increased risk of birth defects or miscarriage with cephalosporin use, including cefixime.
Lactation: Unknown if cefixime is excreted in human milk, but it is present in animal milk. Use with caution while breastfeeding.
Caution in patients with renal impairment, GI disease, or allergy to penicillins.
Use during pregnancy and lactation only if clearly needed.
Prolonged use may lead to superinfections with resistant organisms.
